Please fill out the the Phase II Capital Campaign Pledge Form below to make your pledge for the construction of the DeBriyn Center.  After you fill out all fields, Click SEND to submit the form.  An email will be sent to St. Joseph staff for processing.

PLEDGE COMMITMENT

Your pledge is a commitment to contribute a certain amount to the Capital Campaign through December 31, 2012 when this Campaign officially ends.  For example, a monthly pledge of $50 per month is calculated from the date you submit the form thru 12/31/2012.  If that is 14 months, your pledge will be $700. A confirmation and formal thank you will be sent to verify we understood you well.
